AN OPEN LETTER TO MY PATIENTS
Last July, after 14 years of practice and as a partner in SouthPark Internal Medicine, I decided to separate my practice from SouthPark Internal Medicine. This change was met with great successes and greater challenges. After a year of trying to maintain a separate practice in the same physical location as SouthPark, I have decided that the best thing for myself, my staff and my patients is to make a complete and total separation. As such, I will be moving to a new location effective August 2, 2010.
My new location is:
9218 Kimmer Drive, Suite 106
Lone Tree, CO 80124
My new phone number is:
303-768-0900
My fax number is the same:
303-791-8152
Other than a physical location change, I plan to continue the same, high quality practice to which you have become accustomed. I would appreciate it if you would follow me to my new location. You will be happy to know that ALL of my staff (Cora, Linda, Lisa, Nancy, Jan and Sue) will be coming with me. I think you will find continued friendly, patient-centered care in my new location.
One major change in my new location will be laboratory drawing. Due to lack of reimbursement for this service and a lack of space, I will no longer be able to have your laboratory work drawn in the office. Instead, your laboratory will be ordered electronically and you will not have to pick up your laboratory paperwork in the office. You will go directly to a draw station for your blood draw. More information regarding laboratory drawing can be found here.
I realize that many of you may have questions about my decision and about how the move to a new location may affect your medical care. You can get answers to frequently asked questions here. If your questions are not answered there, feel free to contact me directly.
I understand that the different location may pose undue burdens to some of you. I will be happy to provide records to another physician if you choose to leave my practice. Please click here to download and complete a release of records form.
I have enjoyed my years affiliated with SouthPark and I wish Dr. Burkett the best.
